Dr. Ajibike Fadekemi Oyewunmi
Ajibike Fadekemi Oyewunmi is a Consultant Obstetrician and Gynaecologist at Lagoon Hospital, Lagos. She holds a Bachelors Degree in Obstetrics & Gynaecology, Obafemi Awolowo University, Ile-Ife, Nigeria and is a Fellow of the  West African College of Surgeons. She is currently the Quality Improvement Manager and a member of Lagoon Hospitals Management. Dr. Oyewunmi has had over 10 years experience in  Obstetrics and Gynaecology, and a certified Professional in Healthcare Quality Cerification Board, USA. She has also had trainings both locally and abroad and has worked in various University Teaching Hospitals in Nigeria. At Hygeia, She is involved in decision-making and management of both High risk and Low risk Antenatal patients (In and Out patients), involved in taking decisions and managing Gynaecological emergencies (Out-patients and In-patients) as well as developing the Organisation’s Quality Improvement and Patient Safety system which includes developing organizational policies and procedures and assisting others to write out policies and procedures for the various departments and involved in the day to day running of the hospital especially as concerns Operations and Human Resources.

Patient Safety in healthcare is about Identifying what works – effective practice, Ensuring that the patient gets it ‐ appropriate use and Delivering care flawlessly – no errors. Further more, there should be no needless deaths, no needless delays, no waste, no needless pain or suffering and no helplessness.

Patient Safety in healthcare is about Identifying what works – effective practice, Ensuring that the patient gets it ‐ appropriate use and Delivering care flawlessly – no errors. What are Health care Errors?  They include Failure to diagnose or incorrect diagnosis, Failure to provide adequate observation, Failure to utilize or act on diagnostic test, Use of inappropriate or outmoded diagnostic test, or procedure, Failure to provide follow‐up, Medication error or adverse drug event, Wrong‐site surgery; surgical error,  Transfusion error.

Key issues in Patient Safety requires responsibility for leadership to create environment that  encourages error identification and remedial steps to reduce risk of future recurrences, and minimizes individual blame or retribution for the involved staff and for the reporter, Organizational engagement in proactive systems analysis as an error  revention strategy.
